Subscriptions forms are mailed to all households and businesses in our service area in the summer of each year.  Residents can also print the form (PDF) and mail it to the address listed on this page.

A household subscription of $55.00 per year provides household coverage when an emergency transport is needed.  All family members living at the listed address and non-resident guests or visitors are covered for emergency medical transports, or treatment and no transports.

Your subscription membership is valid for the calendar year, January 1st through December 31st, or from the date your subscription is received, whichever occurs later.  Please sure to notify our office of any change of address.
